What happened in Intermediate section:
We had several faults on the Franks equipment. This led to troubleshooting, phone calls to town, risk of dropping casing in hole and downtime. Fault in hydraulic panel, caused the casing to drop out of FMS slips. Only dog collar saved us from dropping 2 csg jts into the well. Unable to break connection and take back-up jaws on casing coupling, csg tong alignment. Interlock function signal fault.

What Was Planned

Run 9 5/8″ casing using the Franks FA-1 casing circulating tool. This means Franks crew are controlling the elevator, slips, casing tong and the single joint pick up elevator. The driller is only running the casing up and down. The result is that more work load is put on the Franks crew, and more critical equipment is supplied and operated by Franks

What Was Learned

We are exposed higher risk for downtime when Franks is operating and suppling all handling equipment. Need more robust manning, more reliable equipment.

What Actions Are Required

Consider change casing running contract. Get more robust manning and experienced personnell. Get better maintained & reliable equipment.

What Was Done

Stopped operation. Secured casing. Troubleshoot and tried to get support from onshore (not easy in the middle of the night, saturday) . Used back-up equipment.
